强新民,1954年生,在河南省卢氏县横涧乡七寸村卫生所工作40余年,1979年在本县卫校学习中西医结合课程,1984年在洛阳地区卫校进修中医大专班学习,现从村卫所转至洛阳市涧西区第二社区卫生服务中心工作。临床擅长治疗疑难杂证疗效显著,在灵兰中医书友会和河洛医家公众上发表过几十篇文章。

情志病症状很多,属于现在医学精神病范畴,包括失眠、狂躁、焦虑、抑郁等症。

There are many symptoms of emotional disease, belonging to the category of medical mental illness, including insomnia, mania, anxiety, depression and other diseases.

其大致可分为身口意三个类型:

It can be roughly divided into three types of body and mouth meaning:

身——指人的行为做事异常。表现在出现嗜睡,或木僵或打人或杀人放火等不良行为;

Body —— refers to the behavior of people with abnormal work. The performance in the occurrence of drowsiness, or wood or beating or killing, arson and other bad behavior;

口——指人的语言异常。语言表现出语无伦次;

Mouth —— refers to the language abnormalities in people. Language is incoherent;

意——指人的思维方式异常。思维出现胡乱想惑疑心重,疑神疑鬼,或认为配偶有外遇,或认为别人谋害自己,出现恐惧,害怕某某事情会发生。

情志病对家庭对社会都带来的是一种灾难,害人害己,使财产金钱、家庭感情都受到损失。作为一名医生职业,能把情志病治愈,让人心情愉快,家和万事兴,达到人人健康,这是医生的职责和使命。

在当今社会,西医为医学主流,人发生情志病时,首选西医,可以说一半以上的病人以医院为家,终身服药,失去生活能力。但情志病仍是其疑难病,吃药有效,停药犯病的情况普遍存在。

In today's society, western medicine is the mainstream of medicine. When people suffer, the first choice is western medicine. It can be said that more than half of the patients take the hospital as home, take medicine for life and lose the ability to live. But the disease is still a difficult disease, effective medicine, the situation of the disease is common.

我从事中医有40多年,曾治疗精神病千余例,也积累了一些经验。新病可以3个月治好,旧病3年以上或十年以上者,可以用药半年或一年可愈。用纯中药虽然疗效慢,但复发者少,治愈者多。也有少数人不听医嘱,吃药有效后却不坚持巩固治疗,到处访名医,最终没有坚持,甚为可惜。

在这里,我把自己有关治疗经验分享出来,供同仁参考,以图共同提高。

Although the curative effect of pure Chinese medicine is slow, there is less recurrence and more cure. There are also a few people do not listen to the doctor's advice, take medicine effective but do not insist on the consolidation of treatment, visit famous doctors everywhere, finally did not insist, it is a pity. Here, I share my own treatment experience for colleagues reference, in order to improve together.

癫症:多属阴症,见于胡言乱语、心烦,但欲寐,赖床但又不能入眠,头脑昏昏沉沉,走路不知方向,有时如痴如呆不言语,有时会四肢不停乱动,自言自语,说话声音低嘟嘟囔囔让人听不清。

狂症:多属阳证,大声吆喝骂人,狂奔、登高甚至上房顶,翻墙、打人、毁物。

辨证施治:

diagnosis and treatment based on an overall analysis of the illness and the patient's condition:

八纲辨证:

analyse and differentiate pathological conditions in accordance with the dight principal syndromes:

癫症:多属阴症、虚症、寒症。

Epilepsy: mostly Yin, deficiency, cold.

狂症:多属阳证、实证、热证。

Crazy ness: mostly positive, empirical, hot.

六经辨证:

analyse and differentiate febrile diseases in accordance with the theory of six pairs of channels:

癫证:多属少阴病

Epilepsy syndrome: mostly shaoyin disease狂症:多属阳明病  少阴病

Disorder: mostly to Yangming disease and shaoyin disease

脏腑辨证:

癫症:多属心肝脾肾

治则:

癫症:疏肝理气,补益心脾,温补肾阳。

狂症:清心包胆胃之火,泻胃肠之积滞。

常用方药:

柴胡加龙骨牡蛎汤。

Chaihu and keel oyster soup.

柴胡、黄芩、人参、半夏、生姜、大枣、龙骨、牡蛎、铅丹、茯苓、大黄、桂枝。因为铅丹有毒故不常用,临床用磁石代之。根据患者症状轻重不同,可将用量分为3类。

Bupleurum, Scutellaria baicalensis, ginseng, pinellia, ginger, jujube, keel, oyster, oyster, lead Dan, poria, rhubarb, cassia branch. 重症:

柴胡120g,黄芩45g,人参30g,半夏60g,生姜15g,大枣12枚,龙骨100g,牡蛎100g,磁石60g,茯苓50g,大黄30g,桂枝10g,另加枣仁30g,黄连10g。

grave: Bupleurum 120g, Scutellaria baicalensis 45g, ginseng 30g, pinellia pinellia, ginger 15g, 12 dates, keel 100g, oyster 100g, magnet 60g, Poria 50g, 30g rhubarb, 10g cassia, 30g jujube kernel, 10g Coptis.

均为每日一剂 水煎600毫升分3次服

柴龙汤是我几十年治疗狂症经验方,原方来源于《伤寒论》。

其理论根据为:肝主条达,肝藏魂,肝主情志,肝胆相照,肝主谋虑,胆主决断,其病理为肝血不足,肝木不条达,易患抑郁。也有气郁化火、肝郁气滞、胆火不降、上扰心神。

There are also qi yu fire, liver stagnation, gallbladder fire does not fall, disturb the mind.

方中柴胡疏通三焦,条达肝气,气郁得散。黄芩清肝胆郁热。大黄泻胃肠之火,导热下行,邪有出路,心火下降,心神得安,谵语自止。茯苓化痰安神。桂枝强心阳。龙牡磁石重镇安神定魂魄。人参大补元气,养五脏定心神。半夏降胃化痰,与茯苓皆能化痰安神定志。姜枣调脾胃,保津液,能防苦寒与金石之药伤脾胃。

Pinellia pinxia stomach phlegm, and poria cocos can reduce phlegm calm mind. Ginger jujube tune the spleen and stomach, baojin fluid, can prevent bitter cold and stone medicine to hurt the spleen and stomach.

另外也有时方治狂之方药可参考,有生铁落饮、黄连温胆汤、蒙石滚痰丸、安宫牛黄丸。

我分享一下大黄的用量,大黄为治狂要药,《伤寒论》仲景治狂善用大黄,对谵语惊悸狂躁用大承气汤、柴龙汤,抵当汤、桃仁承气汤皆用大黄。

常用量大黄每剂药可用30克,与诸药同煎泻下作用较缓,后下则泻下作用较强些,但泻几次后再继续服药好像有抗药性,只是大便稀并不大泄,谵语症状逐渐消失。

马某、女、17岁、就诊日期:1993年4月、狂躁,半月日夜不眠,不大便,狂奔骂人,不穿衣,不避亲疏,当时娘家妈非常着急怕女儿之病难愈,公开招聘谁家小伙若不嫌我女儿有病,请医生把病看好后就给谁家当媳妇,这家姓宋的男子家有点穷,认为说媳妇有点难,请余上门诊疗,我大胆开出超出常人想象的药量。

大黄120g犀角粉9g木香9g 水煎一日服完。

患者服药后,泻出棋子样大约十几粒硬便,狂躁、谵语症状大减,之后改为柴龙汤治愈。

另外柴龙汤对于狂症与癫症交替出现也同样有效。

失眠为常见症状,兼心烦心悸舌淡苔薄白脉虚。所有精神病皆有失眠症状,热象不明显者,可以养心安神方剂治

主方:养心汤

Main side: Yang Xin soup

方药:人参、黄芪、甘草、茯苓、茯神、当归、川芎、柏子仁、半夏曲、远志、肉桂、五味、枣仁。

Prescription: ginseng, astragalus, licorice, poria, poria, fu god, Angelica, chuanxiong, cypress seed, pinellia sinensis, Yuanzhi, cassia bark, Wuwei, jujube kernel.

根据病情可加入龙骨、牡蛎、夜交藤,合欢皮之类加强安眠。

肝郁气滞型:

常见心情郁闷,面色青、胸满。自觉气撑两胁,厌食,失眠、小心眼遇到小事也过不去坎,惑疑心重,怨恨心生起,事事不如意,根据肝主情志理论为依据,以疏肝解郁为治则。

第一方:释忧汤。

方药:香附、甘松、柴胡、郁金、藿香、半夏曲、菖蒲。

Prescription: incense, sweet pine, buhu, bujin, yujin, patchouli, pinellia song, calamus.

此方柴胡条达肝气,香附理气,郁金解郁,甘松、菖蒲芳香入心开窍醒神,藿香、半夏降胃气化痰湿之浊,痰去神安心情开朗,因此释忧汤为解郁神方。

第二方:解忧消愁汤。

方药:柴胡、当归、白芍、白术、甘草、生姜、薄荷、陈皮、半复、神曲、麦芽、山横、香附、郁金。

此方由逍遥散、二陈汤、焦三仙加郁金香附组成。

Prescription: bupleurum, Angelica, white peony root, atractylodes, licorice, ginger, mint, tangerine peel, half compound, Divine Comedy, malt, sweet, mountain heng, fragrant, yu gold. This side is composed of free and loose, two Chen soup, jiao three Xi'an plus tulips.

功能:解郁化痰理气消食。吃了逍遥散,心情道遥自在,快乐无比。

Function: solution, phlegm, qi and digestion. Eat free and unfettered scattered, the mood is far away and comfortable, very happy.

在我的临床经验中,感悟到治疗抑郁症最有效的方子应该是释忧汤。此方虽然药味少,但疗效可靠。

In my clinical experience, I realized that the most effective treatment for depression should be the relief soup. Although this prescription has less medicinal flavor, its curative effect is reliable.

2021年3月份治疗一例抑郁症,王某、女、50岁,发病2年,头昏,整天迷迷糊糊,走路不知东西南北,二年前由于婆母娘经常找事,自己又不敢反抗,日久肝郁气滞,胸胁苦满,默默不欲饮食。服释忧汤加获神远志,全方每味药各10克,服15剂后自觉心情开朗,再听到婆母娘唠叨时,心不烦了,医嘱要继续服15剂以善其后。

另外还有一例。李某、女、26岁、患抑郁症8年,服此方30剂后精神焕发,想去考驾照,想去上班。这家老两口就这一个姑娘,医生救一个人就是救了一个家庭。

心肾阳虚型:

Heart kidney Yang deficiency type:

心悸、失眠、恐惧、烦躁、但欲寐、懒床症、坐卧不安、眼不想睁、手足冰冷、不欲饮食、身如木僵状、转侧困难,不想说话,问之不答、如痴如呆,状若木偶,生活失去自理.

治则:大补阳气

第一方:桂枝甘草龙骨牲蛎汤加味:

主治心阳虚,以恐惧为主症,心神不安。如人将捕之感,或疑遭人陷害。属惊悸之症。

此方一般服药30剂可愈。

病例:一男子,郑某、40岁,2020年春节发病,此人以按装晾衣架为业,由于疫情小区被封,楼门也被封,半月不准下楼,心情郁闷,得了恐惧症。一听到疫情严重心里害怕扑通扑通乱跳,日夜不安,不敢登高,面临失业。2020年3月就诊,服此方30制病愈。

Due to the epidemic closure of the community and the door of the building, he was not allowed to go downstairs in half a month, so he felt depressed and got phobia. In March 2020, I visited 30 and took 30 days to recover the disease.

第二方:大回阳饮:

主治肾阳虚,以如痴如呆,状若木偶、四肢冰冷,舌色青苔白脉沉细弱为主症。

干姜30g附子30g炙甘草30g肉桂30g

根据症状可加淫阳霍、当归、吴茱萸、鹿角霜。

病例:刘某、男、40岁,就诊日期:1990年4月,发病1年余,只觉浑身僵硬,不可转,如木偶状,皮肤肌肉如板状,皮肤粗糙四肢阳经部位有蛇鳞状图样,冰冷、舌苔白、舌质青,脉沉迟。

处方大回阳饮:干姜、附子、炙甘草、肉桂、当归、吴茱萸各30克,服药60剂而愈。

此症属于古中医记载附体病,症状复杂多样化,是人在运气低时或身体虚弱时,或者遇到挫折时祸不单行,受外来的邪灵干扰入侵,使自身灵魂受到控制失去了主宰的功能,出现了替身,改变了原事的生活规律,行为语言思想被第三方指控,做事像换了一个人似的,非常荒唐,能使思想改变有自杀行为,也有的人说话声音改变为另一个人声音,西医称为幻觉幻想幻视。

这类病人在精神病中占三分之一,在治疗上不论中医西医都束手无策,导致终生服药成为一个废人,这类病在中医经典里都有记载,都有治疗方法。但现在从医者受科学理论影响,认为中医经典有封建迷信部分,提出取其精华,去其糟粕,对用肉眼看不见的现象所记载的附体病症状否认,这类方药也逐渐面临失传。只能对这类病人实行趟平、病人与魔共存。

This kind of disease is recorded in the traditional Chinese medicine classics, and there are treatment methods. However, under the influence of scientific theories, doctors believe that traditional Chinese medicine classics have feudal superstition, propose to take the essence, discard the dross, and deny the symptoms of possession diseases recorded by the phenomenon invisible by the naked eye, and such prescriptions are gradually being lost. Only to this kind of patient practice trip flat, patient and demon coexist.

这些症状至今在古医藉中仍有有蛛丝马迹可寻,《神农本草经》有服中药后轻身,久服成仙记载,《伤寒杂病论》记载有如见鬼状,如神灵所作等症状,《辩证录》记载有附体病症状和中邪的分类。每种邪都有相应的方药,比如逐蛇汤、断媚汤。

我常用方药为:邪崇汤:

I commonly use prescriptions for: evil worship soup:

方药组成;节菖蒲、鬼箭羽、桃奴、当归、川芎、人参、茯神、麦冬、远志、苍术、甘草、生姜、辰砂、雄黄、木香、金银同煎。

就诊日期2000年5月10日,自诉右半身冰凉已3个月,右半身与左半身冰凉之界线非常清晰——前面面部与胸腹正中线与后面头颈与脊柱正中线为界线——左半身暖右半身冰凉。病人形容说,这好比木工用墨打过线一样,标准不差一分。鼻梁与眼睛左边与右边感觉都不一样,左暖右凉。

我对此症治疗并没有经验,也没看到过医书记载身凉的病例。心想王清任治疗半身瘫用补阳还五汤,半身瘫与半身凉都是身的一半出了问题,瘫的半身肯定也是凉,病机相同症状轻重不同而已,皆是元阳之气不足所致。
